Third ET entity presenting with JAK2 wild type prefibrotic (right panel) primary megakaryocytic and granulocytic myeloproliferation (PMGM, left and middle), which is characterized by a hypercellular bone marrow due to dual myeloproliferation of granulopoiesis and dense clustered enlarged immature dysmorphic megakaryocytes (left and middle panels) with bulky (bulbous) hyperchromatic nuclei (arrows), which are never seen in JAK2 wild type MPL515 mutated ET and also not in the prefibrotic JAK2V617F mutated ET, prodromal PV, EMGM and trilinear PV entities.
